A meeting about a child or young person who has a learning disability and/or autism and who is in a mental health hospital or learning disability hospital, or is at risk of being admitted to one of these hospitals. The aim of the meeting is to make a plan to prevent the young person going into hospital, or to plan their move out of hospital if they are already there. It should include all the health, social care and education professionals involved in the young person's care.
