When the actions of a public body such as a local council cause injustice. You can ask the Ombudsman to investigate if you believe that an organisation has failed in this way. There is no exact legal definition of 'maladministration', but examples include an organisation failing to act in the way it should, giving out misleading information, or not doing what it said it would.
