Owen was a 19yo with severe learning disabilities. He also suffered from Pica (an eating disorder characterised by a tendency to eat non-edible substances).

Owen's carer repeatedly raised concerns about him eating twigs, leaves and blue paper towels with his social worker and school. Despite this, Owen was left unsupervised and died after choking on blue paper towel.
