Kim died after being attacked by his son, who was under inadequate care from the MH team.

They had not carried out MH assessments for the son in accordance with s.13 MHA, in that (among other things) they had disregarded the information repeatedly provided by Kim and his wife. A proper MH assessment would have led to Kim's son being sectioned and prevented from attacking Kim.
