Use case:
Emergency hospital admission
When someone gets taken to hospital by ambulance, there may be a huge amount of important information that needs to accompany them. Some of it may be held in their Summary Care Record or Shared Care Record, but what about their "About Me" record, their communication needs, or the records kept by their care workers? Not to mention data from care tech?
